Model health certificate for blood and blood products from equidae for purposes outside the feed chain [4(C) Equivalent]

To be eligible for this certificate, a facility must be listed for [and the materials must meet the requirements of] the Regulation 142/2011 as amended “CHAPTER 4(A) Health certificate for the import of blood and blood products from equidae to be used outside the feed chain, for dispatch to or for transit through the European Union”.

Advisory regarding commercial documents e.g. company certificates of origin or analysis:  The EU and Great Britain have indicated that consignments of blood products will be rejected if accompanied by commercial documents containing more specific information than the below certificates.  APHIS strongly recommends that exporters not include commercial documents (or other paper work) with the consignments that make any claims regarding the origin of the blood products or about other information addressed in these certificates.  The pre-lined out versions below contain the most specific assurances APHIS can provide for these consignments.

The health certificate and notes for completion are two separate documents.  When submitting the certificate(s) for endorsement to the service center, it is required to include both the certificate and the notes for completion with the appropriate line-outs. The notes section is not required to accompany the certificate or consignment but is provided with line-outs to assist exporters and endorsing officials.  

In section I.28 of the certificate, for "Species (Scientific name)": enter one or more of the following terms as appropriate: Equus cabalus, Equus asinus, Equuis cabalus*asinus.

    To utilize these versions, the inappropriate text must be lined out prior to endorsement for both the certificate and the notes of completion. The text must be lined-out consistent with the pertinent versions above.
