ANNEX 1D:  Model Official Certification of plants producing milk replacers/milk products for animal feed intended for export to Israel

This document is only for the plant’s production of products (for export to Israel) that are not licensed by the Center for Veterinary Biologics (CVB).

Each consignment to Israel must be accompanied by the lot specific ANNEX 2D:  Model of a Veterinary Certification to Accompany milk replacers/milk products for animal feed intended for export to Israel.  In some cases prior to import, the Israeli importer may be required to submit the “ANNEX 1D:  Model Official Certification of plants producing milk replacers/milk products for animal feed intended for export to Israel” to the Israeli competent authorities to gain permission to import materials with the ANNEX 2D.  Exporters should have their importers contact their local Israeli authorities for details.  If the ANNEX 1D is required, manufacturers should contact their local Veterinary Services veterinary service center for details on arranging the necessary inspection. 

Select the appropriate version by the testing option to be utilized for products exported to Israel with the ANNEX 2D (excluding CVB licensed products):
