Update Your Contact Information

Maintaining your contact information is an integral part of your participation in the program.  We rely on having your current e-mail address and other contact information to be able to notify you about your renewal and other pertinent animal health issues.  You may update your contact information by:

  1. Completing the VS Form 1-36A and mailing to the appropriate NVAP Coordinator

  2. Accessing the VSPS system, and submitting a change online
