How to Have Your Accreditation Reinstated

If you did not renew your accreditation by your assigned accreditation renewal date or, for veterinarians accredited prior to February 1, 2010, did not elect to participate in the National Veterinary Accreditation Program by October 1, 2011, your accreditation has expired. Do not perform accredited duties until you receive notification of reinstatement of your accreditation from APHIS. Veterinarians who perform accredited duties while their accredited status has expired will be subject to such criminal and civil penalties as are provided by the Animal Health Protection Act (7 U.S.C. 8301 et seq .) or other applicable Federal statutes or regulations.

If you know the six digit National Accreditation Number which was issued to you sometime between 2010 and now, your instructions for reinstatement of accreditation are identical to your instructions for renewal of accreditation, found here: Renew my accreditation

If you don't know your six digit National Accreditation Number or you don't know if you have a six digit National Accreditation Number, contact the NVAP Coordinator of the State where your business address is.
