     Questionnaires:

    • General Swine Farm Questionnaire (GSFQ)
      • This questionnaire was used by NASS to collect data from July 1 through August 15, 2012. It was administered either by face-to-face interview or by computer-assissted telephone interview (CATI).

    • VS Visit questionnaire
      • This questionnaire was used by APHIS to collect data from September 15 through December 15, 2012. It was administered by face-to-face interview.

    • VS Visit questionnaire show card
      • The interviewer was instructed to show the respondent the list of antibiotics, medications, and biologics referred to by questions in the VS Visit questionnaire. This attached sheet also included a series of questions designed to determine the PRRS status of the breeding herd.

    • Blood collection record (BCR)
      • This form was completed on sites that permitted the collection of blood samples. It links the bar code, which identified the blood sample, with the site. It also captures additional animal-level information on specific pigs that were bled. There were two versions of the blood collection record: one for late finisher and one for sow/gilt.

    • Fecal collection record (FCR)
      • This form was completed on sites that permitted the collection of fecal samples from the floors of pens containing late finishers. It links the sample ID with the site. It also captures additional pen-level information.

    • Primary grain collection record (PGCR)
      • This form was completed on sites that permitted the collection of fecal samples from the floors of pens containing late finishers. It links the sample ID with the site. It also captures additional pen-level information.

    • General Swine Farm Questionnaire (GSFQ) for operations with fewer than 100 pigs
      • This questionnaire was used by NASS to collect data from July 1 through August 15, 2012. It was administered by computer-assisted telephone interview (CATI).
