Model health certificate for milk, milk-based products and milk-derived products not for human consumption [12 Equivalent]
To be eligible for this certificate, a facility must be listed for [and the materials must meet the requirements of] the Regulation 142/2011 as amended “CHAPTER 2(A) Health certificate for milk, milk-based and milk-derived products, not intended for human consumption for dispatch to or for transit through the European Union”.

Milk and Milk-Based Products to the European Union for Livestock Feed
Colostrum and colostrum products:
APHIS cannot endorse certificates for exports of colostrum or products derived from colostrum to the European Union (EU). The EU has certification requirements for these products that cannot be endorsed by APHIS. APHIS has asked the EU to modify these requirements for products intended for animal consumption, and discussions are ongoing. Exception: Colostrum (intended and labeled for consumption by pet animals not including livestock species or horses) may be included in certain pet foods produced and exported under the Chapter 3(A) or Chapter 3(B) Health Certificates. Ovine/Caprine (Sheep/Goat) milk products for feeding to farmed animals (other than fur bearing animals such as mink):
The EU requires certification for these materials (if imported for feeding to farmed animals other than fur bearing animals such as mink) that is not available for U.S.-origin materials. At this time, ovine/caprine-origin dairy products are ineligible for export to the EU for use in feed for farmed animals (other than fur bearing animals such as mink).
Chapter 2(A) Health Certificate for Commodities (Not including ovine/caprine materials for feeding to farmed animals [other than fur bearing animals such as mink] nor any colostrum):
The below certificates are for materials that are milk-based and packaged in new containers. For other materials, please go to the bottom and select the “other” options.
Please select the appropriate certificate below based upon the milk-origin ingredients included in the product(s) to be exported, and the processing method(s) utilized. The health certificate and notes for completion are two separate documents. When submitting the certificate(s) for endorsement to the service center, it is required to include both the certificate and the notes for completion with the appropriate line-outs. The notes section is not required to accompany the certificate or consignment, but is provided with line-outs to assist exporters and endorsing officials.
