Submission of Nominations - General Instructions
The following are general instructions on how to nominate someone for the PPQ Deputy Administrator's Safeguarding Award.
1. Refer to and consider PPQ's mission statement as you decide whether to nominate someone for the award.
2. If you are a PPQ employee, then you must submit your nomination through the nomination SharePoint site. If you are reading a hard copy of these instructions and you need the SharePoint address, then please write to PPQ-DAAC@aphis.usda.gov.
3. If you are not a PPQ Employee, please use the following AWARD FORM TEMPLATE above to submit your nomination.
4. You may submit a hard copy or an electronic copy of your nomination.
Mail a hard copy of your nomination to:

You may e-mail a copy of your nomination to PPQ-DAAC@aphis.usda.gov.
5. Nominations will be accepted from May 7 until 5 p.m. Eastern on May 30, 2014.
