The following are general instructions on how to nominate someone for the PPQ Deputy Administrator's Safeguarding Award. 1. Refer to and consider PPQ’s mission statement as you decide whether to nominate someone for the award. 2. Use the following form to submit your nomination. Part A:

3. You may submit a hard copy or a soft copy of your nomination. Mail a hard copy of your nomination to: Clinton Campbell, Ph.D. Or, fax him a copy of your nomination at (253) 874-1109. You may e-mail a soft copy of your nomination to PPQ-DAAC@aphis.usda.gov. If you are a PPQ employee, then you can submit your nomination through this SharePoint site. If you are reading a hard copy of these instructions and you need the SharePoint address, then please write to PPQ-DAAC@aphis.usda.gov. 4. Your nomination must be received by March 15, 2012.
