Before you leave for military duty, please review the important information on these web sites regarding your Federal civilian benefits, and take the necessary action(s).

Provide a copy of your military orders to your supervisor and ask that the SF-52, Request for Personnel Action (LWOP – US) and orders be submitted to Human Resources.

Federal Employees Health Benefits (FEHB):

The USDA pays the employee share of the FEHB premium for up to 24 months for employees ordered to active duty in support of a contingency operation.  If you or your family will not be using your FEHB plan, you may cancel enrollment by sending a completed SF-2809 (http://www.opm.gov/forms/html/sf.asp) and orders to your servicing Benefits Specialist.  You may reenroll in FEHB within 60 days of returning to Federal civilian duty.


Federal Employees Dental and Vision Insurance Program (FEDVIP):

If you are enrolled in a Federal dental or vision plan under FEDVIP, please contact BENEFEDS at 1-877-888-FEDS (1-877-888-3337) or email Service@BENEFEDS. com for information about premium payments while you are in non pay status from your Federal employment.

When you return to Federal civilian duty, the post-1956 military service deposit may be required to credit this service for CSRS/FERS retirement.

When you return to Federal civilian employment, please contact your servicing Benefits Specialist within 60 days to obtain current information about benefits decisions you need to make.
