- The SF-2817 (PDF; 79Kb), Life Insurance Election Form, was revised in June 2000, and older versions are not acceptable.
- Please note...There are NO regular open seasons for the Federal Employees Group Life Insurance program. Opportunities to increase life insurance coverage are limited. You may decrease your coverage at any time.
