In order to facilitate the personnel document distribution and processing, we encourage you to attach HRO Form 444c (rev 10/09) Personnel Document Transmittal Form.
Information such as Program identification, employee’s name, SSN, and effective date, and the servicing Processing Associate (PDF) can be a useful recording tool as well as helpful to the mailroom and Processing section.
Note: HRO Form 444c can be completed on-line and printed. The data you enter in this form can not be saved or submitted at this time. In order to complete this form on-line you will need Adobe Acrobat Reader version 3.0 or higher. Click on the link below to get this software or upgrade your software.
All documents (unless otherwise stated) have been created in Portable Document Format (PDF) and can only be viewed by using Adobe Acrobat Reader.
